It is well known that pancreatic cancer is a cancer with a very poor prognosis. Even in stage IA pancreatic cancer, the 5-year survival rate of patients is only 14%. Once pancreatic cancer reaches stage III or IV, the 5-year survival rate is sharply reduced to 3% and 1%. "The aggressiveness of pancreatic cancer is notorious and its prognosis is extremely poor," commented Dr. Andrew Epstein, ASCO's invited expert. "A treatment that significantly improves patient survival is a major victory in treating the disease."
In order to find such a method, the researchers conducted a large number of experiments. In a clinical trial called PRODIGE 24/CCTG PA.6, the researchers recruited a large number of patients with non-metastatic pancreatic ductal adenocarcinoma (PDAC), the most common form of pancreatic cancer, accounting for 90% of all cases. %. These patients underwent surgery to remove the tumor. At 3-12 weeks postoperatively, a total of 493 patients were randomized into two groups, one receiving gemcitabine and the other receiving new chemotherapy mFOLFIRINOX (modified FOLFIRINOX). The latter contains four different chemotherapy components, including oxaliplatin, leucovorin, irinotecan, and 5-fluorouracil.

The study showed that median disease-free survival was significantly higher in the mFOLFIRINOX group than in the gemcitabine group (21.6 months - 12.8 months) during the median follow-up of 33.6 months. In the overall median survival period, the former is more than 20 months (54.4 months – 35.0 months). Under the additional survival benefits, the side effects of chemotherapy are also controllable.
"Our study shows for the first time that adjuvant FOLFIRINOX chemotherapy is more profitable than standard chemotherapy," said Dr. Thierry Conroy, principal of the study. "In addition, we are happy to see results better than expected."
According to the plan, the researchers will continue to explore the best time for chemotherapy to understand whether patients can receive chemotherapy before surgery, reduce tumors, thereby reducing the risk of tumor micrometastasis and increasing the probability of tumor being completely removed by surgery.
